In order to get access to a laboratory this form must be completed and submitted to:
Phil Watson
Laboratory and Technical Services Manager
Thrn. 1175 (Machine Shop)
Once the form is submitted and approved, your swipe card will be enabled to access the requested lab. Only one form per group is required.
If you have been assigned laboratory space, complete this form and post it by your experiment. This is helpful in case of an emergency where a student must be contacted or if there is a problem with facilities or equipement.
